Oral GLP-1 BMF-650: Up to 3.2% Weight Loss in Four Weeks
Topics: GLP-1 & Metabolic Health

Jeff's Daily Pick. A new GLP-1 pill took off up to 3.2% of body weight in four weeks with no titration ladder. Investors still sent the stock down 30%.
In the United States, Biomea Fusion announced on October 13, 2026 the Phase 1 results of BMF-650, an oral small molecule GLP-1 receptor agonist for obesity, in a release also filed with the SEC. The randomized, double blind, placebo controlled GLP-131 study (NCT07223216) enrolled overweight or obese, otherwise healthy adults, with a single ascending dose part followed by five multiple ascending dose cohorts.
The weight numbers
In outpatient cohorts 1 to 4, mean weight reduction at day 29 ranged from 1.5% to 3.2%, compared with 0.1% for pooled placebo. At doses of 200 mg or more once daily, mean weekly weight loss was about 0.97 kg. A fifth cohort, treated in confinement with 200 mg for one week and then 400 mg for three weeks, lost 6.1% (6.2 kg) by day 29. That cohort's two placebo patients lost a similar amount after eating about a third of the food served, which makes the 6.1% hard to attribute to the drug.
Safety and design
Among all BMF-650 treated participants in the multiple dose cohorts, there were no severe or serious treatment emergent adverse events, no dose changes and no discontinuations due to adverse events. The company says it designed the molecule for less acute GLP-1 receptor activation in the gut, to improve tolerability and allow a single step titration to the 400 mg target, instead of the months long titration typical of the class. "Our objective in designing BMF-650 was not to maximize weight loss compared to other oral GLP-1s," said Thorsten Kirschberg, who leads the program.
What it shows / what it doesn't
- Shows: in four weeks, the pill produced modest dose related weight loss over placebo with no serious adverse events.
- Shows: a one step titration to the target dose was tolerated in this small study.
- Doesn't show: how it compares with other oral GLP-1 drugs over months; Phase 1 cohorts are small and short.
- Doesn't show: a clean result for the 6.1% cohort, given its placebo arm.
- Status: company topline data; full results to come at a medical meeting.
The market reaction tells the competitive story. Biomea shares fell 30% to 87 cents in early trading the next day, from a $1.26 close, The Catalyst Brief reported. The company had $35.2 million in cash at the end of June, funds operations into the second quarter of 2027, and plans to advance BMF-650 through a partnership. The field it enters already includes approved and late stage oral options; our piece on orforglipron against semaglutide sets the bar.
